Leak problem

We have a carport that was turned into a den we had noticed that
some of the carpet was getting wet when it rained. We tore up the
carpet and padding we found the wood used to build the floor was
also wet we have take that part out.. We found that it looks like
that between the floor and wall that was where the leak was. What can we do correct this poblem?
Thanks

Still need pictures, so others can see what you are dealing with. My guess I would say, that it was not properly done, is why you are getting water intrusion, which means a grade issue up to the old pad. The wood is most likely all rotted, that was used as plates against the raw concrete.
