laminate wood flooring over concrete

Hello all,
We are wanting to replace our carpet with laminate wood flooring but are kinda confused.
I've visited a lot of websites trying to find how to do this and Im stuck between two options.
Website 1 said: Make sure the concrete is clean and completely level. Lay a layer of 6mm poly over the concrete to eliminate moisture. Then start laying out the laminate.
Website 2 said: Make sure the concrete is clean and completely level. Place plywood on top of the concrete. Then place padding on it. Then lay the laminate.
Could someone please tell me which advice to use.
Also, what can I use to clean the concrete?

The concrete floor you are about to lay the flooring is a basement floor or an above grade room? There is a difference and your answer will allow us to answer your question more precisely.

The room we are replacing the carpet on is our living room an above grade room.

Doesn't matter. a 6 mil poly will work just fine. no need for additional plywood at all.
I disagree about level. It will work if it is out of level on the entire room, however the room needs to be FLAT.
