How to level/fix subfloor before hardwoods?

After ripping up the underlayment the subfloor has some chunks taken out. I'd rather not rip it up considering the granite that's on the counters and it's under all of the counters. I plan on sanding it to remove splinters but how should I go about leveling fixing the holes? I heard that self leveling stuff is hard to work with.

Is it out of level or not flat?
If it's flat do not worry about those bad spots, just go over the whole thing with 3/8 A/C underlayment plywood.
Then 1/4 tile board set in a bed of thinset.
Are you sure this floor will even support tile? What's the free spans, spacing and width of the floor joist?

Ah, I should've mentioned I'm removing the underlayment so it's level with the other hardwood. I'm going to be putting 5" hardwood floor over this so I'm worried about the holes.

Hardwood will easily ride right over missing chunks---Unless you have large areas with entire plys missing you should be fine----
