Floor over concrete garage floor

Single car garage with concrete--slight fall (can live with that); will have Pergo-type flooring when finished. Would like to add some form of insulation between the concrete and the 'Pergo'. 1. Is there some form of insulation that can be put on the concrete and the 'Pergo' floated over it? If not, 2. Is building a 'frame' base on the concrete and putting on plywood and then the 'Pergo' (putting in insulation between the framing boards) the only other solution? 3. What type of insulation could I use, then, which would also have the vapor barrier?

Floor over concrete garage floor

A vapor barrier pad for lam will work just fine. The floor can be unlevel, but MUST be flat.
Please do not drive the car on it
