Cement board is flush with plaster

The cement board is going in my shower wall. would it be a problem If my cement board and plaster are
Flush and I still have to put the tile on? Would this look bad if it sticks out 1/4-1/2inch?

May want to go back and look at your post, it's not all that clear what your trying to do.
If it's what I think it is then you use cap or bull nosed tile so it leaves a rounded edge where the tiles end.

Bullnose tile or a schluter trim.
