bathroom floor

I just tore up the tile in my hallway bathroom and plan on replacing it with a vinyl floor. The problem is that when I tore up the tile, it almost looks like there is concrete under it and there are places where the "concrete" has come up and is no longer level. What type of subfloor should I put down to remedy this before I lay the vinly??
I also plan on removing the vanity and replacing it with a Pedastal sink, putting in a new toilet and doing a "Tub surround: on the bathtub and tile...

Tile should be mounted on a concrete substrate.
That needs to come out.
then reinstall at least 3/8 plywood AC for a vinyl substrate.
then install vinyl.
