How can I tell the amps need of this 5 hp motor?

FLA. full load amps. This is the normal load and startup can be much more such as SLA or LRA on some data plates

I'm thinking there should be a surge amp rating for start up.

Figure 3-5 times rated for instantaneous startup. I'd use a 30 and 10gauge.

True, the momentary will peak that high but you size it for running amps. The breaker is inverse time so it will allow a brief surge. The higher, the quicker it pops.
