Can an appliance that's 110 operate safely on 220, or will it damage it?

NO, you can not operate a 110v appliance on 220

For an interesting example of the trouble you can get into operating a 110 volt electric dildo at 220 volts, see the movie Top Secret with Val Kilmer. Generally not a good idea.
