No, you do not need to strip it. Apply one coat of Zinsser Cover-Stain. This is a fast-dry, oil-based primer.

Others have advised you about proper ventilation; be sure your fan is big enough and clean enough to do an adequate job.

Too much work! I would try drilling the spots out and patching with joint compound before taking it all down to original plaster.

We used to have the same problem too but after we added a good ceiling fan (Panasonic has the best bathroom ceiling fan BTW!), the problem went away almost immediately. Just my 2 cents.
