Whole in Crawl Space Wall? Water Problem? - Picture!

Hello,
My wife and I recently purchased a house. We knew there was a slight moisture problem in the crawl space, but that it was due to inadequate downspouts and the lack of a vapor barrier. We had these two issues fixed before closing, but after another big rain, we noticed still a small amount of water coming into the crawlspace. Water was not on the vapor barrier anywhere in the crawlspace, except for the lowest point.

Anyhow, I was curious to find the cause of this to correct it before any major problems, and I came across this rather large hole where the dirt crawl space floor meets the cinder block. I am almost certain this is where water is entering, as just to the RIGHT, where the crawl space floor slopes down greatly, is where there was a small amount of water on the vapor barrier.

I am a little upset that our home inspector did not see this, however what do you think can be done to fix this? It looks sort of like water has eroded the soil which might be causing the soil. I was thinking of either using expanding foam or sakrete cement mix to patch it, but how do you patch cement on a non-solid dirt floor?

Finally - does this look serious? Like, are there serious foundation issues at work here?

Any thoughts are well appreciated!

This is the transition from the full basement to the crawlspace. I have a similiar transition with a poured concrete foundation. The house was built in 1949 and I don't have any issue with it. I've lived here 19 years.
