se-630 Phone Jack

Does anyone here know where to find a phone jack like this? I searched online and couldnt find anything like it. It is a jack for a system of warehouse phones I am working on. I know they put this in around 1999 so if it isnt being made anymore is there anything comparable?

Looks like an normal 6 position RJ11 phone jack only it has punch downs instead screws for the connections.

Or is it an 8 position RJ45 LAN connection with only four wires?

I think you can buy both at Home Depot.
