Water pipes through a raised foundation wall


I want to add an outdoor shower with both hot/cold water. My house is on a raised foundation so I was hoping to run the pipes through the wall and connect up. I'm in San Diego so freezing weather is not an issue, and the pipes will be at least 6" above ground level so ground water won't be an issue either. I want to make sure what I do is to code. I was planning on tapping in to my hot/cold copper pipes under the house using a T, come out through the foundation, then up the exterior wall to the adjustment valve? Do I need any shutoff valve under the house? What's the best way to sleeve the pipe through the wall? thanks

Line the hole with PVC pipe.
I would install ball valves under the house in case the valve needs service.

Can I run both hot and cold pipes through the same PVC sleeve, or should it be 2 seperate holes? How big a PVC pipe should I use, and how do I seal the space in between the PVC and copper pipe? Should the PVC pipe be flush with the foundation wall? thanks
