Inclosed Back Porch Footing Replacement


I have a 1910 house that we are currently making improvements on. One of the areas that will need to be resolved is our back porch. The inclosed and heated porch (8' x12') was built on a perimeter of concrete 12"W x 18"D which has cracked and twisted in a number of areas.
So I need to resolve this issue. I would like to keep the porch intact. Could I replace the old foundation with concrete piers and build off of them. Any ideas or approaches to my problem would be appreciated.

Pictures, Pictures, Pictures...You description left alot of information out that is relevant to a legitimage answer....
