How to Replace water damaged outside wall

My husband and I pulled up old carpeting and found water damaged plywood in front of the sliding glass doors. After pulling out the plywood to replace we found rotted wood under the sliding glass door so bad that we can see and feel the tyvac paper and siding. What do we do now?

You will remove the door and the siding under and around the door(as needed). Replace the damaged structure and reinstall the door and siding properly,as one or both were not done so originally.
