Do I need a form for the concrete pier or just fill up the hole I dug?

I dug three holes to pour concrete piers in my crawlspace to help support a load bearing wall as per the description of the structural engineer I consulted.

Is the earth my form? or am I required to make a form?

The earth is good enough.--Build a wood form to take care of the above ground elevation.
