Is the vapor barrier on the floor seal to the stemwall?

If there's enough relative humidity in the air and the ground temperature is cool enough you will get condensation on top of the plastic regardless of what you do.

The only way to effectively eliminate that would be to eliminate the source of the moisture by sealing up the crawlspace and insulating the stemwall and converting it to a conditioned crawlspace

I have this problem too. What is a stemwall and please define "conditioned crawlspace." Thank you.

Here you'll find much information, including a lot of instrumented studies, and ways to remediate problems:

Starting searching the "Insulation" section of the forum and you will get all the information you would ever need there as well.

Craw space fans work in some parts of the country. In other areas, introducing warm air into a cooler crawl space causes the moisture in the air to condense, wetting the surfaces in the crawl space and making things much worse.

Although a lot of work, closing (and sometimes conditioning) the crawl space is often the best answer ... depending on the prevailing climate where the home is located.
