how thick should the slab be, should it be reinforced with mesh or rebar?

It should be whatever your code officer and architect or engineer say, and if you haven't already heard about "frost proofed shallow foundations" suggest you google that phrase and read up on how to save a lot of concrete

Check your heating degree days for required rigid foam board at perimeter:

Soil conditions and the plans will dictate required use of steel, as mentioned.
