Wiring lights on vaulted ceilings

Existing home has only floor lamps to light it up but I would like to put in actuall hanging lights. How do get a light hung on a vaulted ceiling? You basically have no attic to work with in this situation.

My wife works for a telefony company and a friend works for a security company. A lot of their install techs do sidework, have all of the tools (very specialized and expensive) and can put wiring through a ticked off bobcat starting at either end.
Sorry, forgot the guys that do home entertainment systems. My bad.

like the others said, you have to fish the wires to where you want the fixtures, and down to the switch locations, and over to whereever you are going to get power from (you might want to hire an electrician for this, and they might even need to cut a couple of holes)

Two words.

Cut and patch.

Wait...that was three words.
