Wiring Emerson Electric 115V Motor


I have an old Emerson Electric Motor S60CXBRV-3211 115V, 5.2A 1725RPM that I want to use with my little 4" Beaver Jointer. Does any one have a clue on how to wire it? It has 4 spade terminals marked 1,2 and 4,5. I will need it to go CCW looking at the pulley.

Wiring digram on the back of the cover plate?

I've been looking, that's why I posted here. Emerson Electric no longer exists, it was taken over by US motors, which is now a brand of Nidec Motor Corp. They don't have a listing for this motor.
