Secure NM cable to plastic electrical box


My electrical box for my outlets is plastic and it has the partial punchout places where the cable is supposed to be threaded through.

The problem is that the partial punchouts came all of the way out when I was pulling the cable into the box. Now there are a bunch of holes in the box and no clamp to secure the cable to the box.

What am I doing wrong such that the punchouts are coming all of the way out?

Is there still a safe way to secure the cable to this box?

If this is a single gang plastic box, all that is necessary is to secure the cable within 8" of the box. Just don't leave any unused opening in the box.

It is a double gang box. Does that change things at all?

Should I cover the open holes in the box with electrical tape?
