Running low voltage & data wires

What kind of code requirements are there with running low-voltage data lines like phone, cable, ethernet, security systems, speaker-wires, etc?

Aside from using proper wire type & gauge, is there a rating for in-wall use?

I believe low-voltage lines must be kept a certain distance from regular power lines. What is that distance?

In order to avoid RF interference they should be shielded or kept 12" away from power lines. If they cross a line it should be at a 90 degree angle. Shielded means a regular wire run inside metal conduit, or that it comes with a foil shield built in.

The only rating is for Plenum use, which means inside air ducts. Everything else is fine for running inside walls.
