I will try to answer that question however I will throw both Americian and French verison in the mix so it will get ya off guard with it
1) 200 A is most common for upgrading service but exsting 60/100/200 in USA side in France it will varies from 32 amp all the way to 360 A service { the voltage will be diffrent than what the states will have }
2) I don't useally make any mistake on that one due I always double , triple check it all the time.
3) Just about anything can go wrong so prepare for it and know how to deal with it properly it is not a issue as long you plan it ahead with it.
4) Oui it can but the NEC or CEF or POCO regulations will trump it so it will varies a bit depending on POCO and type of code you have to use.
5) Oui we do have few 120 volts 30 amp service but it getting rare ditto in France we have couple old 127 volts 16 /24 amp service left
6) Oui quite few in my area in USA side both single and triphase and few 360 amp service in France { very limited on this one }
7) Oui., both in USA and France { 32 amp }
8) The best answer is get someone who is qualifed and experncie with this.
9) Load caluation and experince { this part you will NOT skim on this matter }
