Phase out.

Why would a phase go out on a three phase electrical panel?

Phase out.

Could be a utility company problem or a problem in the facilities panel.

Phase out.

If its not a faulty switch / fuse or a loose connection,
Then it could be a blown fuse only on one phase,
Possibly the phases are not balanced and there
is too much om one phase.
There will be a fuse for each of the three phases.
