need junction box, but no wall cavity

I need to install a junction box on an interior wall to install a wall sconce. Problem is that there is no cavity behind the drywall to install the box. there is a triple wide 2x10 header stacked onto a double top plate at the level the box needs to go.
Short of having to cut into the header to recess a standard junction box is there another way to accomplish the same thing?
I was thinking maybe a round light pan like you might see on an exterior light fixture. this thing basically mounts onto the surface of the wall, they the sconce cover plate covers that.

The pancake box pic posted abave can only hold one cable. You will also need some way to recess the cable.
