My Kidde Combination Smoke/CO Alarm will not go through the test cycle on AC - why?

I installed the Kidde Combo Smoke/CO alarm (KN-COSM-IB) and when powered up (green light lit) it will not go through the test cycle. However, if I turn off the AC and use battery only the test button/test cycle works. What have I done incorrectly?

Thanks,

Dan
