What is this fan being used on? (make/model) <-- ask them
...or call Grainger and ask if they have the cross index

The number you provided produces no results on the Grainger web site. Write down everything on the nameplate and post it here. Make sure you write everything down. We cannot cross anything without something to work with. You know you could not get anywhere with that number. Why did you just post the useless number. We need all the nameplate information.

Use full words to communicate.

Is "mod" modification or engineering change order?

Or is "mod" model?

And then what do you want? If model, to buy a replacement?

If modification, do you want a copy of the modification for installation?
