Floor Box Installation

I plan to install a floor box in concrete which will provide an outlet in the middle of a living room floor. The instructions that came with the PVC floor box are unclear. Is the floor box and the conduit that is attached to it just laid on ground level and secured with rebar? The box itself doesn't seem deep enough to be buried any at all. I have had experience burying conduit under concrete, but have never installed a floor box.

the box needs to be set to be either flush with the finish floor or just a little bit under the finish floor. maybe 1/8 inch.

Secure any way you can.

If you run conduit to the floor box the nec depth is Zero inches. The concrete people may want more to keep the concret from cracking.

Secure any way you can, or be there when they pour to keep the box in the place you want.
