Electrical Boxes Flush with drywall

Should the electrical box be flush to the exposed surface of drywall?
(i mean the "painted" side of the drywall)

Is code very stringent about this?

Ideally, they should be flush or slightly recessed to the finished drywall.

No more than 1/4" setback in non combustible wall surfaces such as tile, drywall, etc.. Must be flush with combustible wall surfaces.
