crooked outlet

I hope someone could clear this up for me. I've read that outlet boxes can be no more than a 1/4" below the wall face, but does this apply to all types of walls?? For instance, wood is much more combustible than drywall and ceramic tile is practically impossible to ignite with sparks. Also what if the outlet is near a corner where the spackle causes one side of the box to be recessed, sometimes more than a quarter inch?

If the surface is combustible, The box must be flush, or project from the wall. If non-combustible, 1/4". "Add-a-depth" rings are made for this.

The NEC says "concrete, tile, gypsum, plaster or other non-combustible surface" 314.20
