Ceiling boxes and insulation

I just want to see if I have something straight... I understand clearance to insulation with recessed lighting unless it's IC rated, and heat-generating fixtures in general. Does this apply to AC wired smoke detectors, ceiling fans and surface mounted ceiling light fixtures? Or are these okay to have insulation in contact with the electrical box in the ceiling?
