Water leak

I have an older home 1948 with a newer furnace. I seem to have water bleedung through the concrete about two feet below where the metal pipe connects into the chimney. I have a new filter in it just not sure.why the wall is bleeding

Condensate in the chimney from the furnace???

Is the chimney on an outside wall?
Is the vent into the chimney single wall or double?
Do you have a liner in that chimney?
Is the space between the furnace chimney vent and the chimney wall open at the top of the chimney?
What kind of furnace do you have? Btu's on the rating plate? Overall height of the chimney and the size of the furnace vent pipe? Length of the furnace vent between chimney and the furnace? Inside diameter of that chimney?
Do you have an old chimney clean out, outside that is open?
Is the furnace room kept cold?
