Venting Bath and Kitchen fans

Currently renovating a ranch. Bathroom and kitchen are back to back, and the bath fan is vented through the attic out towards the side of the house. When the siding was done a few years back they blocked up the outside vent, which will also have to be re-installed. I am planning to install a ducted hood fan in the kitchen and run the hose parallel to the bath vent hose. My question is how to connect the 2 to the vent? Do I use a simple T-connector at the end of the run, closest to where the outside vent will be? Any suggestions are appreciated.

Venting Bath and Kitchen fans

Bath and range vents can not connect together per code. Range vent needs to be hard piped with no skrews.
