Hrv intake duct insulation soaking wet

The insulation is soaking wet from the outside wall to about 15 feet in. I drained the water by cutting a hole in the plastic, once there was no more water coming from the insulation, I sealed the hole with tape. Now about a week later the insulation is soaked again. I can not see any other holes in the plastic. The outdoor temperatures in December has been a high of 0 Celsius and a low of -18 Celsius. Any suggestions would be greatly appreciated.

Clear the snow away from the intake and exhaust hoods. Make sure the exhaust is not recirculating into the intake. Sounds like it sucked in snow and moisture and defrosted inside the pipe. Seen it happen several times.
