dDistributing heat from a fireplace insert.
 
I primarily heat the house with a fireplace insert at one end of the house. Our bedroom and bath at the other end of the house stays very cold. I would like to distribute some of the excess heat with a fan and ductwork to the bedroom and bath. I would need approximately 40 feet of ductwork and a fan. The area to heat is 350 square feet. House is well insulated.

What size and type of ductwork would work? What type of fan would be best? Can I go ceiling to ceiling?

If you put the duct work in the attic. You'll want at least an R8 insulation value.

Depending how warm/hot the area with the fireplace is. You will need a fan that moves 400 CFM or so.

ductwork size
 
What size ductwork should I use? 4", 6", 8"?

Would I use an inline type fan? Does it go at the beginning, middle or end of the run?

10" might need 12" depending how straight the run is or isn't.
Fan can go at teh beginning of the run or end, or middle. Where ever is more convenient for you.
