Patchy dead grass

Last spring I had a few dead patches of grass. I went out removed the old dead grass (really it was more of a woody type that grass) and used a small rake to loosen the soil and patched it with Scott's Patchmaster. It grew nicely and now it looks very similar to what it did before I did anything last year. Am I over reacting, or is there some other issue.

Here are two pictures a before and after. You can view them lager by clicking on them.

Dead spots on grass is either from animal visits or a lawn disease. Search the net for pictures of both, may help determine which.
