Lawn not growing well


Looking for a bit of help.

My grass I planted last spring isnt doing very well. I should have done a soil test first, but didnt. It is a new construction house and I tilled up the ground and had black dirt hauled in leveled and seeded.

The grass is very clumped and very sparse inbetween. I have fescue, bluegrass and rye planted. I live in northern MN.

I did a soil test last night and it was very alkaline and very low for N,P and K.

My question is what do I need to add to my lawn to help it out?
Can I just broadcast it or do I need to rake it in or work it into the soil?
