Weeds and grass are tough to permanetly get rid off in a gravel driveway. It's a constant struggle. You can use a chemical herbicide in spring and fall, or if you want a more eco-friendly approach you can use salt and boiling water. Corn gluten as a preimergant might work. You could try raking them out as well. You could do nothing but mow as low as possible, and just add gravel on top, which might choke out a lot of it by not allowing it to get sunlight. No matter what you do, be prepared to do it or a combination of things a couple times a year.
As for keeping gravel out of the grass, edging or some type of curbing are really the only thing i can think of.
