Extending a patio...

Hello - I need to pour concrete into an area that goes under a door threshold (for a screened in porch) and is also attached to a brick patio. I believe I've read enough to do this, but now we (she) wants to include a landing area with 4 18" square brick pavers outside the doorway. In all, the area is about 16 sq ft. The $1M question is, how do I get the pavers at the proper height for the doorway? I don't think I can raise them up with sand before pouring concrete. The sand would 'eak' out, loosen the pavers and make the small concrete pad too thin. Any advice would be welcomed. Thanks.

form & place the bagg'd crete under the threshold, remove the form, add gravel, compact, place h/d pavers, & brush in holding sand ?

don't forget rebar & do NOT make your threshold pour less than 8" x 8" or be prepared to have it crack in the middle .
