In a new construction installation. The old window is totally removed from the opening and replaced with a new unit.
In a replacement installation, the frame of the old window is left in place. The opening is cleaned of the old sashes and parting strips and the replacement window is installed in the old frame.

AS Ron said, and some replacements come to you with nailing flanges, which you can cut off easily. Cons., you need the flanges.
