Install bathroom plumbing,

Core drill would be the best way to get a round hole. HD or any rental place should have one.

It can also be done with a series of holes close together with an impast drill and just knock out the plug but it makes for one messy hole and will blow out the concrete where the drill comes through.

Install bathroom plumbing,

You can also get bid(s) from concrete coring companies. The costs are also less if you agree to break-up and dispose of any debris for them.

Install bathroom plumbing,

You will also have to sleeve the pipe that is going to come through the wall with another pipe, but as the others said core it,that is the surest way I know how to do it.
