bath remodeling

Thank you for taking my question. I have a long narrow half bath (only 4'2" wide) I would like to turn into a full bath which requires moving fixtures. Due to limited space, I want to install a 32"x32" shower in the toilet space, move the toilet in the sink space and shift the sink in a new space along the same wall. I do not want to tear up the concrete slab bathroom is on so I would like to buildup the shoer and toilet area of the floor with trusses to run the drainage pipes. Is this possible? Also, how do I extend the toilet drain to new location?

This doesn't sound like you can move all these fixtures around without busting up the slab. Waste pipes need slope for drainage, especially the toilet. Too far away and things just sit there.
