Cabinet Hinges

I have some old cabinets with self closing hinges. 2 of the hinges are bad and need replacing. I have been looking the same hinges or ones that closely look like them but there are ones issue with the ones i find. As seen in the attached picture there is the aprt of the hinge where 3 hole are drilled, with the single hole at the edge. Te hinkes I find have the holes reversed. Does anyone know where I can find my style of hinge?

Is there a name on the hinge?
If you can't find anything at one of the big box stores, I'd try this:

You can always drill new holes. The hinge will hide the old ones.
