Sealing where sill plate meets foundation

Hi, Not sure if this is the right section for my question, i apologize if it is not. My ranch home was built in 1954, there is no membrane between the sill plate and the poured foundation. There is cement on the inside where they meet. I'm trying to seal things up and fill those cavities with insulation. I noticed some areas were very drafty and found that the cement is loose and separating, so I would like to pull the old loose stuff out and redo. Is there a specific type of cement I should use? Or are there other methods for doing this? What do you suggest? Thanks for any input!

even w/o pics, this is how we do it,,, clean out all the loose conc & mortar then insert closed cell backer rod into the cavities on top of your insulation,,, seal w/a SEALANT, NOT CAULK ( to save on gas, you can buy all the stuff @ your apron/vest store - masonary section aisle

you could also use foam-in-a-can not to be confused w/hair-in-a-can altho the cann'd foam can be messy compared w/tooled sealants,,, 100% silicone works well, too

Thanks for the replies! Here are a couple pics as requested. itsreallyconc, a little confused with how you said put it in the cavity on top of the insulation, did you mean foundation? joecaption, it's not really falling apart as much as it is loose, i can grab it and lift it.

Just looks like grout or mortar that was put in as a stop gap for air filtration and it'shrunk.
