Replacing Small Cracked Porch/Slab (with pictures)

Hi Everyone,

I've attached a picture of the small cracked porch in front of one of my door entrances. I would like to replace this.

I am not sure why it cracked. Perhaps there is no re-bar or other type material in the porch? Maybe this is why overtime it just cracked?

Besides concrete, what would be the material needed to reinforce the concrete so that this does not happen again?

Anyone have any good resources that I should follow in building a new porch or concrete slab?

How thick should the slab be considering right now it's 42 x 36 inches?

It most likly failed because the propper prep work was not done under it before poring.

For a slab that small you never really need to use rebar. If I were pouring a slab like that, I would make it at least 4-5 inches deep.
