New Patio - Pour over existing or demo?

Very rarely will I pour over a garbage floor/slab w/o tearing it out first. Subgrade is critical here in our freeze/thaw region. If the first slab failed, the second likely will as well if the subgrade isn't adequate.

As for the 4", I was talking about a 4" buffer vertically, which there obviously isn't room for most of the time, so we generally tear it out.......
