How to stub off an old stone and mortar foundation

I've torn out an old crumbling foundation that was holding up my porch, I plan to replace it with footings and basically building a deck for a porch. My question is that I need to pour some concrete to stub off the portion of the foundation that is intertwinded with the old house's foundation. The picture will illustrate what I mean. Basically how should I tie the existing foundation into the concrete I plan to form and pour on top of it? I was planning to clean up the existing foundation, wash it well, then use an additive for cohesion. I hope to build the forms down to a couple inches below grade and a couple inches wider than the existing foundation. I should also note that this "foundation stub" will be used as the footing for the back of the future porch/deck.
