Cement hearth

Had to remove false floor in front of fireplace because prior owner did't replace properly, had some of the false floor burned. Cut away false floor, now down to concrete, its a basement family room. Made a mold for hearth, what can I do to ensure bondage of the new 4 inch hearth to the cement floor?

Cement hearth

How thick do you plan on poring it?
What are you putting over the concrete?
Clean the floor, remove any sealer, use concrete bonding agent.
